New Patient Registration

When you move into the local area you can request to join the practice by visiting the surgery and requesting a new patient information pack. This contains a welcome letter from the Patient Services Manager, the patient information booklet, a new patient questionnaire and a GMS1 form.

pdfRegistration Form

When you return these completed forms, reception staff will register you as new patients on our clinical system and a request to retrieve your clinical notes from your previous surgery will be made.

We ask that all patients attend the surgery for a 10 minute health check with the nurse to record relevant health information about you as it can take up to 8 weeks for the practice to receive your clinical notes.

If you are on regular medication your repeat items will be added to your clinical notes if we have the information but you may also need to see the doctor.

Please remember to update any other health organisations such as hospitals with your new address details and give them the name and address of the practice so that we receive all correspondence relating to you when you attend appointments elsewhere.

Change of Address

If you change your name, address or telephone number, please let our receptionists know by calling or writing to us. If you move outside the practice area you will need to register with a new practice and your clinical notes will be transferred.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
